By submitting payment for my child’s participation in SBRD2 Recreation Programs and Athletics, I acknowledge and consent to SBRD2’s use of certain information about my child, which may include: Image, Age, Grade/School, Honors and Recognitions at Local, State, and/or National Levels, Organizations recognizing student-athlete achievements, Athletic Team,Programs/Rosters, News Columns/Media I agree not to hold SBRD2, its employees, sponsors, or affiliates liable for any present or future claims, including but not limited to property damage, personal injury, or wrongful death, arising from my child’s participation in the program. This waiver applies to claims resulting from the ordinary negligence of the parties mentioned, regardless of when or where the incident occurs. I understand that participation in the program involves inherent risks, including serious injury. By allowing my child to participate, I accept these risks and certify that my child is in excellent physical health and capable of engaging in physical activities. I also grant permission for my child to receive emergency medical treatment if necessary. I agree to indemnify and hold SBRD2 harmless from any claims arising from my child’s participation in the program or related activities. I understand this document is intended to be as broad and inclusive as Louisiana law permits. Should any portion of this agreement be found invalid, the remaining provisions will remain in full effect. I understand that no refunds will be issued once SBRD2 has ordered player jerseys. I have read and fully understood this waiver, and I acknowledge that I am voluntarily giving up certain legal rights by signing.
